National Awards
Policy for Application Procedure for CHHA National Awards
- Nominations for these six national awards are to be submitted to the CHHA National Office on the enclosed forms. Advance nominations will be received by e-mail or fax; however, original forms must be submitted with signatures.
- Nominations must be submitted at least two months prior to the presentation date. The CHHA National Office will announce the deadline for submissions each year.
- The nominator and two supporting CHHA members must sign all nominations. The nomination description should be no more than 500 words.
- Please ensure that all required information, signatures and supporting documentation is included with the nomination submission. Omissions and/or errors could lead to delays and even to your candidate missing the opportunity for selection.
- The CHHA National Office will send an acknowledgment of receipt to the nominator. If the nominator does not receive the acknowledgment within one month of submission, he or she should contact the CHHA National Office.
- The President will submit the nominations before the Board of Directors for consideration one month before the desired date of presentation. If a member of the Board of Directors feels that he or she is in a conflict of interest regarding a nomination, he/she may withdraw from the award selection process. The Board of Directors may appoint a replacement.
- The CHHA National Office staff will prepare the awards for presentation at the National AGM/Conference.
- Presenters can be so designated by the Board of Directors.
- Nominators are responsible for the submission of nominees each year. Any declined nominee is not automatically considered for the following year.
- The decision on awards is final. The President is not required to disclose any reason for declining nominees. As members of the Executive and Board are volunteers -- like you -- their decision should be accepted and respected.
- There will not necessarily be a winner for each award each year. If there are no nominations submitted for any one category, or if the nominees do not meet the outlined criteria, then there will not be a winner for that award in the given year.
- CHHA is not responsible for expenses incurred by award recipients to attend an awards event. If the recipient(s) is/are not able to attend the award presentation, the Board of Directors and the nominator will make other arrangements.
